Family Resource Specialist

Fremont Healthy Start, a program of East Bay Agency for Children (EBAC), is a dynamic, ethnically diverse non profit organization that is dedicated to assisting the community in an culturally and linguistically appropriate manner. We assist families with health insurance, information and referral, application assistance and other case management services. We are looking for someone who is compassionate and sensitive to the needs of their community. Applicants must speak Mandarin/Cantonese and English.

The Family Resource Specialist performs intensive community organizing and outreach services to nurture and strengthen at-risk Mandarin speaking families in the Fremont community. Special focus areas of this position include outreach to Mandarin speaking families with children ages 0-18 to gain access to health care, help promote school readiness to families with school-age children, assist families new to the area with resources as well as with case management.

QUALIFICATIONS and COMPETENCY FACTORS

  1. Multilingual/multicultural in Mandarin/Cantonese/English
  2. Good interpersonal and communication skills.
  3. Sensitive to the needs of the community with genuine desire to work with people in a compassionate and empathetic manner.
  4. Ability to work with various public agencies, a culturally diverse staff and culturally diverse parents.
  5. Ability to work independently and to exercise professional discretion and judgment.
  6. Ability to drive with valid driver's license, auto insurance, and access to use of an automobile as needed.
  7. High school diploma or general education degree (GED); or one to two years related experience with case management or in the human service field.
  8. English/Mandarin/Cantonese language skills that include the ability to read and interpret documents such as applications for Medi-Cal, Food Stamps, Cash Aid, Dept. of Rehabilitation, etc. Ability to write routine reports and correspondence. Ability to speak effectively and express ideas clearly with individuals and before groups.
  9. Cultural sensitivity and awareness.
  10. Basic computer competency specifically Word and Excel.
  11. Experience working in and with the Mandarin speaking Fremont community preferred.
  12. Preference may be given to applicants with previous experience in case management, and/or community services.

RESPONSIBILITIES (Those with an asterisk are essential duties.)

  1. *Assists Mandarin Speaking families in gaining access to health care by providing outreach, education and information.
  2. 2*Ensures school age children are enrolled in school.
  3. *Reaches out to the most isolated families in the community to assist them to utilize available public institutions and services by providing bilingual and bicultural interpretation and translation.
  4. *Outreaches to immigrant and non-English speaking families to help understand cultural orientation and to develop cultural competence.
  5. *Provides case management and organized documentation of cases.
  6. Undertakes and follows up on assessments, service plans, referrals, consultations, access to services, and all other activities related to case management.
  7. *Inputs information into database and maintains all required data for billing and client tracking purposes in a timely manner.
  8. Mobilizes community participation in school-site activities.
  9. *Provides community organization and advocacy services.
  10. Assists with community education presentations and parent training, periodically.
  11. Performs other duties as assigned (evening presentations, health fairs, etc).

WORKING CONDITIONS and JOB SETTING

  1. Work is in a routine office setting, with no special physical requirements.
  2. No required lifting.
  3. Noise tolerance for nearby train a few times per day.
  4. Periodic driving required.
